Lincoln County Community Health Improvement Plan (CHIP) & Healthy Minds Lincoln County

The purpose of the the Lincoln County Community Health Improvement Plan is to mobilize people, organizations, and stakeholders to collaboratively address issues that impact health. The ultimate goal is to improve the health of Lincoln County. This plan asks for action that will lead to positive change in four health focus areas:

  • Social Determinants of Health and Health Equity
  • Mental Health and Substance Abuse
  • Oral Health
  • Nutrition and Healthy Foods

The Healthy Minds Lincoln County Coalition, a participant in the development of the CHIP and also led by the Lincoln County Health Department and in the past by UW Extension, works to address mental health and substance abuse.  This coalition was formed through a merger of the Lincoln County Drug Free Coalition and the Lincoln County Mental Health Coalition in July of 2018.  Mental Health and substance use are related issues and many individuals wish to work on both so forming a new merged group was deemed to be an effective way to proceed by membership of both coalitions.   The mission of Healthy Minds for Lincoln County Coalition is to bring the community together to empower people through education and resources to support healthy choices regarding substances and social and emotional well-being.
