Club Record Book Forms
You can download the Club Record Book forms you need to your computer. The Club Secretary is encouraged to turn their club record book in for award selection at the end of the year. The information that should be included in the Club Record Book includes the forms below. You may download, save these forms to your computer to use throughout the year to keep a record of the activities and progress that the club has been involved in throughout the 4-H year (October – September). You will want to engage other officers, where appropriate, in completing forms that relate to their officer position (i.e. Treasurer would complete the Club Financial Record). If you wish to use an alternative format, this is acceptable as long as it includes all of the same information (i.e. Club Leader could print adult leader list from 4-H Online system).
